DISD GT Digital Referral Forms

  • Digital Parent Referral Form - to be completed and submitted online
  • Digital Parent Referral Form (Spanish)

 

  • Secondary Student (Grades 6-11) Self-Referral Form - students in grades 6 through 11 may refer themselves for GT screening. Additional information will also be requested from teachers and parents to accompany self-referral forms.  

 

  • DISD Teacher / Educator Referral Form – any DISD educator may refer a student for GT consideration by submitting this digital form.

  • Sept 23, 2025  – Oct 8, 2025: Accepting referrals

  • Nov 3, 2025 – Jan 23, 2026: Testing for all referrals

  • Feb & March 2026: GT Committee meets for placement decisions
